We suggest arriving 15 minutes early with your photo id to allow enough time to check-in

Please note
Testing is by appointment only – do not bring any guests with you
Photo id is required – it must be current
Testing time will be 2 hours
O this includes the time it takes to fill in the information at the top of the answer form
O those who are unable to complete the test in 2 hours can submit the completed portions and return at the next offered session
Tests will be supervised for the entire two hours
Providers must pass the exam within 2 tries
Substitute caregivers can try to take the test repeatedly
O there is no limit to how many times a caregiver can take the test but it is best to have your caregivers educated on the material beforehand
This is a one-time certification
O keep your certificate forever
O we do not keep a copy of it
Interpreters are not allowed
O staff are required to be literate in English both oral and written communication
No assistance from others can be provided during the test
Phones, bags, notebooks, snacks, etc…. Will be stowed on a table away from the test-taker during the test.
if you leave the room you will not be allowed to return
Please make sure to wear a mask.
